CQC issues paper certificates and electronic certificates according to the type of certificate media required by customers. The certification client (applicant) has not applied for a paper certificate of product certification, and if there is a need for a paper certificate after the issuance of the electronic certificate, please follow the following process:
1. Please download and fill in the “ Application Form for Certificate Reissue for Paper Certificate”, then send the original form to CQC. For more information, please refer to the application form.
2. We will send the paper certificate after receives and verifies the documents. If there is a problem, we will contact you.
3. If you in need of on-site services, please download and fill in the "Application Form for Reissuance of Product Certification Paper Certificate" and bring the original to the customer service hall of CQC for on-site processing.
4. There is no charge for a paper certificate reissue. For CCC certificates and issued paper certificates, if they are lost, they must be executed in accordance with the Certificate Lost Reissue application process.
