If the certificate was lost, please take the following steps.
I . Please download and fill in the “Application Form of Lost Certificate Declaration”, then send it to CQC. The form must be sealed by the applicant organization. If the applicant is a foreign company, it must have the signature of the principal.

II. Charging standard
1. Application for lost announcement takes 200 Yuan each certificate.
2. Application for reissue certificate takes 400 Yuan each one, including the lost announcement fee.
III. Charging way
Please transfer the payment into the following account:

7. After completing the related procedure, we will mail the invoices and related documents to you by the address on “Application Form of Lost Certificate Declaration”.
IV. If you want to pay on the spot, please take with the original paper of “Application Form of Lost Certificate Declaration”.
