In accordance with CNAS-EC-016: 2007 Notes on the Relevant Provisions on Refusing to Accept the Application, Suspension or Revoking of the Accreditation Qualifications of Certification Bodies) issued by the China National Accreditation Service for Conformity Assessment (CNAS) and the requirements of China Quality Certification Centre (CQC) on accreditation risk management, in case that you has identified any of the following major quality/environment/safety issues, please fill in the Report on Major Issue of the Certified Applicant (see the appendix) and deliver it to the CQC branches within 2 days. More details are given below:

I. Major quality/environment/safety issues refer to:

1. The applicant recently experienced major organizational changes, or changes in business direction, or major adjustments in product structure or process and other situations that may affect the operation of the system;

2. The product quality within the scope of certification is unqualified in the national, provincial or industrial supervised sampling inspection;

3. A major quality/environment/safety accident or incident has occurred;

4. A major customer complaint has occurred.
