Appendix:

Application Guide of CCC Certification for Toy Products

Part I: Applicants apply for on the internet

Please log in the CQC homepage at http://www.cqc.com.cn. According to the requirements of CCC Application Guide Online (Please find it on the column CCC on homepage), the applicants should remember your “User Name” and “Password”.

Please pay more attention to the following points when you fill in the Application Form of CCC Certification:

1. “Applicant” is the certification consigner of applying for product certification, and the applicant is usually the one who holds the certification.

2. “Agent Organization” is the organization acting as agent of product certification, which should register and record in the Certification and Accreditation Administration of the People’s Republic of China. The agents and offices which oversea Manufacturers establish in China can be regarded as special situation, and they can also be regarded as agent institutions.

3. “Manufacturer” is the organization controlling the product certification, who should take responsibility for the quality of last product.

4. “Factory” is the whole name of the place to produce or the spot to produce.

5. “Product Name” should be filled according to the sub-category of the product applied, using “/” to separate the product “Type”.

6. The information in “CCC Certification Plan” such as applicant and address, manufacturer and address, manufacturing factory and address and brand should be consistent with the corresponding industrial and commercial enrollment certification, organization code certification, brand enrollment certification. Applicants take responsibility for the English information in “CCC Certification Plan”.

7. “Safe Standard” column should be filled according to the “Inspecting Standard” in the Enforcement Regulation of the corresponding sub-category.

8. CB, EMC, CCIB, and CCEE have nothing to do with the toy product certification.

9. The last two pages in the “CCC Certification Plan” are needed the stamp of the enterprise, and signature of the representative of artificial person.

10. The correct Email information is very important.

Part II:

The engineers in CQC Certification accept the application, then provide the application code (the enterprise should remember the application code), and sent the materials to applicant (the applicant may attain the open document in the customized user interface on website www.cqc.com.cn, or in your email box).

Part III:

Applicants do the unit division on certification product applied according to the requirements of “Unit Division Principle” of corresponding products. If necessary, applicant may contact with the engineers in CQC product certification, discussing the questionable plan of unit division.

Part IV Submitting the Formal Materials of Application

1. Submitting the documental materials related to Manufactory to CQC

□ At the first time to apply, applicant should submit the industrial and commercial enrollment certification (with the stamp of yearly inspection), organization code certification (with the stamp of yearly inspection), original document needed by each one.

□ Being sales companies, importer, applicant also need to submit one copy of the related contracts between the sales company and manufacturer, between the importer and manufacturer.

□ If have agent institution, the applicant should submit one consigning document for agent institution.

□ If the manufacturer is not applicant, it should submit the industrial and commercial enrollment certification, organization code certification of manufacturer, one for each one (at the first time to apply).

□ When the manufactory is neither applicant nor manufacturer, it should submit the industrial and commercial enrollment certification, organization code certification of manufacturer, one for each one (at the first time to apply).

□ At the first time to apply, the applicant should submit one corresponding trademark enrollment certification (for the situation of joint venture companies using foreign trademark, please submit the copy of related agreement).

□ When the representative of artificial person can not sign for the related document and certification with some reasons, the representative of artificial person can appoint one agent to replace him to sign. For the situation mentioned, it should submit one formal document of authorization and consigning document.

□ One survey of factory inspection and manufactory brief (for example producing history, producing capacity, resource, etc.)

2． Submitting application materials to CQC on unit

2.1．Two copies of CQC certification application plan.

2.2．Product Description and Related Technological Document

Please submit two copies of writing product description (with the stamp on product description). Product description is filled according to the requirements of corresponding document.

2.3． Informing Sheet of Testing Report and Sample for Product Type and Style

3． Pigeonhole Requirement for the Application Materials Mentioned

The applicant should put the documental materials related to manufactory together, and ones related to unit together according to the unit, then unite them to submit CQC.

Part V: CQC Auditing for Application Materials

The engineers in CQC product certification audit the application materials submitted by applicant. If have any questions, they will contact with applicants; if the application materials are eligible, they will provide sample list and inform the applicant to the inspecting institution appointed to do the test of type and style.

Part VI Test of Type and Style

Test of type and style will be completed by the inspecting institutions appointed by CQC.

Part VII: Manufactory Inspection (fitting for those who need to do the initial factory inspection)

Under the usual condition, after test of type and style, the CCC initial factory inspection (including date, personnel, etc.) will be organized by Sixth Section of Product. Inspection Section is responsible for and organizes the supervision and inspection once a year.

Part VIII: Treatment of ineligible Products

If it occurs the ineligible situation during the test of type and style, and factory inspection, it is needed to return the former responding phase to reorganization and reformation, until they are eligible.

Part IX: Integration Evaluation, Issued Certification

CQC is responsible for the integration evaluation of the result of the test of type and style, and factory inspection. For those products applying for certification fitting for the requirements, it will be issued CCC Certification.

Part X: Usage of Applying CCC Mark

After applicants attain the CCC Mark Certification, please contact with Mark Distribution and Management Center of Certification and Accreditation Administration of the People’s Republic of China to purchase standard mark or apply for Pressing Certification Mark.
